My unit came with the small cuff, but there have been times that others in the family wanted to take their blood pressure and couldn't because the small cuff would not fit. The large cuff works great on big arms.

This Large Blood Pressure Cuff fits my Omron HEM-711AC home blood pressure meter (as well as other Omron meters). Here are my observations:
Pros -
+ Works Well, gives accurate readings
+ High Quality
Cons -
- Hard to put on by yourself
This is a required accessory for those with larger than 13" circumference arms.
